Sammanfattning
Background: The transition towards good and accessible care and advances in medical technology have resulted in increasingly advanced medical care being performed in patients´ homes. Nurses working in home healthcare encounter complex care situations and manage advanced medical interventions in an environment that is primarily a home and not a healthcare facility. The work is characterized by independence and limited access to support while maintaining safe and high-quality care. Previous research has mainly focused on perspectives of patients and next-of-kin regarding the use of advanced medical technology. Nurses’ experiences remain less explored. Aim: To describe nurses´experiences of advanced medical care in the home. Method: A qualitative interview study with an inductive approach. Participants were selected through convenience sampling. A total of six nurses with experience of advanced medical care in the home were interviewed. The interviews were conducted through video calls using a semistructured interview guide. Collected data were analysed using qualitative content analysis according to Graneheim and Lundman (2004) on a manifest level. Results: Nurses ´experiences of advanced medical care in the home showed that advanced care needs often occurred periodically, which was described as rarely used knowledge. Nurses experienced a great responsibility to update their knowledge and create conditions for providing care. Relationships, collegial support and well-functioning collaboration were important for nurses to feel secure in their professional role. Conclusion: Nurses’ experiences of advanced medical care in the home were characterized by independence and responsibility, furthermore by feelings of loneliness and a problem-solving approach. Relationships, collegial support and well-functioning collaboration were important prerequisites for nurses to feel secure in performing their work.
