Background: Non-specific low back pain is a common cause of disability globally. In the case of long-term problems, rehabilitation aims not only at pain reduction but also at strengthening the individual's function and ability to cope with everyday life despite persistent pain. Exercise-based interventions are recommended as first-line treatment. Psychological factors, such as pain-related self-efficacy, have been shown to be important for rehabilitation outcomes, but knowledge of how different forms of exercise affect this factor is limited. Objective: To describe how pain-related self-efficacy is affected by strength training in relation to other exercise-based physiotherapeutic interventions in adults with non-specific low back pain, from a biopsychosocial perspective. Method: A systematic literature review was conducted in the databases PubMed and PEDro. Randomized controlled trials that included adults with non-specific low back pain and reported outcomes regarding pain self-efficacy were included. The methodological quality of the studies was assessed according to SBU's review templates and the results were compiled narratively. Results: Five studies were included. All studies showed improvements in pain-related self-efficacy over time, with no clear differences between strength training and comparative interventions. Conclusion: Exercise-based interventions, including strength training, can contribute to improved pain-related self-efficacy. The results underline the importance of taking biopsychosocial factors into account in the rehabilitation process.
