Sammanfattning

The current rebuilding of Sweden's total defense exposes significant challenges within the country's decentralized healthcare system. The purpose of this thesis is to examine the goal conflicts between the Swedish State and the Swedish Association of Local Authorities and Regions (SKR) regarding the development of healthcare preparedness. Using a qualitative, actor-centered argumentation analysis, the study investigates how these key actors formulate demands and attribute responsibility. The theoretical framework builds on Alvinius and Hedlund’s mechanisms of organizational inertia, categorizing the findings into inability, unwillingness, and lack of synchronization. The analysis shows that the actors operate from conflicting logics: the State prioritizes national security and equal care, while SKR defends regional autonomy and points to a lack of financial resources. The results indicate that structural inability at the regional level creates an institutional unwillingness to accept unfunded state mandates. Ultimately, this dynamic leads to a lack of synchronization in terminology, organization, and legal frameworks. The thesis concludes that the fundamental clash between central state control and decentralized autonomy acts as a structural barrier to realizing Sweden's total defense objectives.
